SUBJECT: SURVEY ON CITIES' RESPONSE AGAINST TUBERCULOSIS

We respectfully request for your cooperation in identifying the problem areas of our cities in combating tuberculosis, which is to date still one of the major causes of deaths in the Philippines,  by completing the survey on cities’ response against tuberculosis and condition of health facilities in cities.

This is in line with the new project secured by LCP through the assistance of USAID and the Philippine Business for Social Progress (PBSP) to provide technical assistance  for cities to implement the National TB Control Program or the 2010-2016 Philippine Plan of Action to Control Tuberculosis (PhilPACT). The PhilPACT calls for the localization of TB control program, and highlights the role of local government units (LGUs) as the main drivers in combating TB, in accordance with the Local Government Code.

In the first part of the survey, we want to know about your city’s specific response and condition in the implementation of TB program, such as the number of TB-DOTS existing in your city, and availability of some health infrastructure. The second section solicits information on how to effectively implement the program in cities as regards marketing and advocacy.

The survey form can be answered by your City Health Officer/National Tuberculosis Coordinator and Advocacy/Information Officer. You may download the form at www.lcp.org.ph under Documents and send the accomplished survey form through facsimile # (02) 470-7210, attention, Policy Unit or send it via-email at league.cities.philippines@gmail.com. Alternatively, you may click the link also found in the website to fill out the survey form on-line.
